A calendar that includes all appointments, meetings, special events, deadlines and important dates for the caregiver to keep track of and manage.


Planner

A practical, day-to-day instructional manual for meeting your disabled loved one’s needs. The Caregiver Planner holds all of the details that are essential for continuity of care. This includes instructions, notes and insights on scheduling appointments, services, housing, education, employment, life skills, entertainment, social engagement. This Planner prepares the next caregiver to be an effective manager of the loved one’s resources and supports.


Funding Plan

Document Portal

The Funding Plan explains in plain language how the disabled loved one will be taken care of financially and the legal documents and supports in place to execute the desired plans. It can also include compensation for the new caregiver.


Our Process


Meet with the family for a review of the Caregiver Guide, input and revisions before final delivery.


Record Review

Review the most recent individualized education program (IEP), individual program plan (IPP), individualized plan for employment (IPE) assessments, behavior plans, and any other document that will help us learn your child’s needs, routines and life goals.


Planning Interview

Meet with the disabled adult, the caregiver and the next caregiver (provided that person has been identified). During this meeting we will use information gained from the document review to discuss building a team of support, selecting a successor caregiver, establishing funding sources for the caregiver to support the loved one, and independent lifestyle plans.

Final Review Meeting

We organize all of your documents into easily accessible folders and prepare detailed instructions on how to manage services and support your loved one.
